Lee was killed while filming The Crow in 1993.  A series of tragic mistakes led to his death.

Lee was killed when a prop bullet and gun was improperly unloaded, reloaded, and then not checked by the film's second unit.  In the scene being filmed, Lee was supposed to walk into his apartment, find his fiancee being raped and beaten and be shot.  A primer was left in a gun, and when actor Michael Massee fired, a real bullet essentially was fired at Lee, hitting his abdomen and lodging in his spine.  Lee died shortly afterwards.
